Saint Francis Adult Day Health is a medical-model day program for adults who benefit from skilled support, personal assistance, and meaningful engagement during the day.

We serve individuals living with memory loss, chronic health conditions, physical limitations, neurological or cognitive challenges, and developmental or intellectual disabilities. Our safe, social environment helps participants maintain their health, abilities, and independence while continuing to live within their community.

Our compassionate team includes nurses, CNAs, therapists, social-services professionals, and activity staff who work together to meet each participant’s physical, emotional, cognitive, and social needs.

Comprehensive Support Throughout the Day

Participants receive individualized care based on their abilities, preferences, health needs, and established care plan. Services may include:

  • On-site nursing oversight and health monitoring

  • Medication administration

  • Assistance with bathing, toileting, grooming, mobility, and other activities of daily living

  • Physical, occupational, and speech therapy services

  • Social services and care coordination

  • Dementia-certified support

  • Nutritious meals and snacks

  • Transportation to and from the program

Our goal is to provide the right level of assistance while encouraging every participant to remain as active and independent as possible.

A Day Built Around Meaningful Engagement

Every day at Saint Francis offers opportunities to move, create, socialize, and connect. Our activity calendar includes music, art, cooking, exercise, games, current events, community outings, and other enriching experiences.

Activities are adapted to accommodate different abilities and interests, allowing each participant to engage in ways that feel comfortable, enjoyable, and meaningful. These shared experiences help reduce isolation, strengthen confidence, and bring greater structure and purpose to the day.

Meals and Transportation

Transportation is available to help participants travel safely to and from the program while making regular attendance easier for families.

Participants also receive breakfast, lunch, and snacks during the day. Meals provide proper nourishment while creating another opportunity for conversation, routine, and social connection.

Supporting Families and Caregivers

Caring for a loved one can be rewarding, but it can also become physically and emotionally demanding. Saint Francis Adult Day Health provides families with dependable daytime support they can trust.

While their loved one is with us, caregivers have time to work, rest, attend appointments, and manage other responsibilities. Our team also helps families navigate changing care needs, available resources, and long-term planning.

We are here to help families care for their loved ones at home for as long as safely possible—without having to manage every responsibility alone.
