Prevention & Wellness: Supporting Health, Independence, and Quality of Life
Staying healthy means more than responding when a problem occurs. Saint Francis Adult Day Health provides proactive support designed to help participants maintain their abilities, manage ongoing health needs, and reduce the risk of preventable complications. Through nursing oversight, nutritious meals, physical activity, social engagement, and individualized assistance, we promote healthier routines and greater confidence each day.
Key Program Elements
Ongoing Health Monitoring
On-site nurses monitor participants’ health and watch for changes that may require attention. Early recognition of concerns allows our team to communicate with families and healthcare providers before minor issues become more serious.
Medication and Care-Plan Support
Our nursing team administers prescribed medications and follows each participant’s established care plan. Consistent daytime support helps families feel confident that important health needs are being addressed.
Movement and Fall Prevention
Supervised exercise and appropriately paced activities help participants maintain strength, balance, mobility, and confidence. Assistance is available for walking and other daily activities to promote safety and reduce fall risks.
Nutrition, Engagement, and Connection
Nutritious meals and snacks, meaningful activities, and regular social interaction support physical and emotional well-being. Each day offers opportunities to remain active, engaged, and connected to others.
