Our nursing team at Saint Francis ADH provides individualized nursing care and ongoing health monitoring to support each participant’s unique needs. Through medication management, nursing assessments, and ongoing management of chronic and acute conditions, our nurses help promote safety, stability, and overall well-being.
We offer services for the following clinical areas:
Neurological/Cognitive
Nurse monitoring of neurological conditions such as Parkinsons, stroke-related needs, seizure disorders, and other neuro/cognitive changes
Fall Recovery
Support for individuals at risk for falls or recovering from a fall, including safety monitoring, mobility support, and coordination of care with therapy team to promote safe recovery and independency
Cardiac Management
Oversight for conditions such as hypertension, heart failure, coronary artery disease, arrythmias, including vital sign monitoring and ongoing assessment for changes in cardiac health
Respiratory Needs
Nursing oversight for COPD, asthma, and other pulmonary conditions, including oxygen administration with provider orders and home oxygen supply, respiratory assessments, daily monitoring
Diabetes Management
Nursing services for Type I and Type II diabetes, including blood sugar monitoring, medication and insulin administration, and ongoing assessment to promote effective diabetes management
Prevention and Wellness
Proactive nursing support focused on health monitoring, early identification of changes, preventive care, health education, and coordination with healthcare providers to promote overall health and well-being
Developmental and Intellectual Disability Support
Individualized nursing care for adults with developmental and intellectual disabilities, with health monitoring and care tailored to each specific participant
Key Capabilities:
Urostomy, colostomy, ileostomy
Provides skilled ostomy care, education, and maintenance to promote comfort and independence.
Oxygen Therapy
Provides prescribed oxygen support and monitoring to promote comfort, safety, and respiratory stability throughout the day.
* medication administration is for any mid-day medications on case-by-case basis, and nursing services are through primary care providers orders
Insulin Pumps
Supports advanced diabetic care with continuous insulin delivery and monitoring.
J tube, G tube
Provides comprehensive enteral feeding support, including tube care, feeding administration, and patient education.
Foley Catheter
Performs routine catheter care and monitoring to reduce risk and ensure proper function.
Blood Sugar Monitoring
Ensures accurate glucose levels for effective diabetes management and medication adjustment.
Suprapubic catheter
Manages specialized urinary catheters safely and effectively.
Mid-Day Medication Administration*
Ensures scheduled daytime medications are administered safely and accurately.
